We provide culturally appropriate resources that are quick and easy to access. Our goal is to remove the obstacles that prevent people, especially people of color, from receiving the necessary education, advocacy, and resources to manage their health. In many cases, patients only receive case management in dire hospital situations. Community health workers are only available to some people, and finding services independently can be time-consuming. Moreover, caregivers often have little to no knowledge of a patient’s cultural background or illness, leading to communication barriers and implicit biases in treatment. VillageTree Health seeks to address these issues and help patients access the resources to manage their health effectively.
